08. How to Apply for a Military Headstone: The application process is handled through VA Form 40-1330 (Claim for Standard Government Headstone or Marker). The form can be submitted online through the VA's website or by mail. Key documents needed include: a copy of the veteran's discharge papers (DD-214 or equivalent); the name and address of the cemetery where the marker will be placed; the veteran's full legal name, birth date, death date, and branch of service.

The VA ships the marker directly to the cemetery at no charge. Families are responsible for setting fees at private cemeteries, though national cemeteries handle installation at no cost.
